Username and Password Reset Guide
To reset your Frederick Health Patient Portal Username or Password, please follow the step-by-step instructions provided below.
Forgot Your Password?
- From the Patient Portal login screen, click "Forgot Password" located below the Sign In button.

Enter your Username and Email Address, then click the "Reset Password" button.
- If the system does not recognize the email address, contact us via email at Portal@Frederick.Health
- You will receive an email from No-reply-portal@frederick.health.
- Click on the blue link that says "Click here to sign in." located at the bottom of the email message.
- The Patient Portal page will open and your Temporary Username & Temporary Password will be automatically filled in.
- Do not change the Username or Password that was automatically filled in.
- Do not save the temporary Username & Password if prompted to do so by your device.
- Click the "Sign in" button.
- You will now be asked to enter and confirm your new password and answer the security question.
- Click on the "Submit new password" button.
- Success! You will now have access to your Patient Portal account using your new password.
Forgot Your Username?
- From the Patient Portal login screen, click "Forgot Username" located below the Sign In button.

Enter the Email Address you used to set up your account, then click the
"Reset credentials" button.
- If the system does not recognize your email, or if the email is associated with more than one account, contact us via email at Portal@Frederick.Health
- You will receive an email from No-reply-portal@frederick.health.
- Click on the blue link that says "Click here to sign in." located at the bottom of the email message.
- The Patient Portal page will open and your Temporary Username & Temporary Password will be automatically filled in.
- Do not change the Username or Password that was automatically filled in.
- Do not save the temporary Username & Password if prompted to do so by your device.
- Click the "Sign in" button.
- You will now be asked to enter and confirm your new username and password, and answer the security question.
- Click on the "Continue" button.
- Success! You will now have access to your Patient Portal account using your new username and password.
