Anticoagulation Clinic

The Frederick Health Anticoagulation Clinic located inside of the Comprehensive Care Center offers ongoing, comprehensive warfarin (Coumadin) management. Our team of experienced and knowledgeable clinical pharmacists work closely with patients and their physicians to optimize anticoagulation regimens, providing education and ongoing monitoring.

Who should come to the Anticoagulation Clinic?

Patients should be referred to the Anticoagulation clinic if they have been prescribed warfarin (Coumadin®). All patients need to be referred by their family physician or specialist.

Ask your physician to fill out a referral form and fax it to the Anticoagulation Clinic at 240-566-3197.

What should I expect?

  • At every visit, the pharmacist will ask you for your health and medical history, including your eating and exercise habits.
  • At every visit, your INR will be tested to determine if your warfarin (Coumadin) dosage needs to be adjusted.
  • Since everyone responds differently to warfarin (Coumadin®), more frequent visits may be needed to determine the correct dosage of warfarin (Coumadin®) for you. Once the dosage has been stabilized, visits will occur every four to six weeks.

What should you bring to your clinic visit?

  • Your health insurance card and photo identification
  • A list of all medications you are taking, including over-the-counter medications, vitamins, herbals and supplements
